THE BRONX - A man died Monday night after he fell five stories down an elevator shaft at 1749 Grand Concourse.

The man was taken to St. Barnabas Hospital where he was pronounced dead. He was moving into an apartment within the building, police say.

According to police, the man was moving a mattress into the elevator and backed in without looking. The doors to the elevator opened, but it never arrived.

There are four elevators in the building that have received 40 violations dating back to 1991, according to the Department of Buildings Web site. The most recent complaints were placed in Jan. and Feb. of this year.
