Dream's story was behind the "Dream's Law" in New York.
The law was signed by Gov. Andrew Cuomo in 2019.
It requires access to safe and continued home care for patients with a central venous line upon discharge from the hospital.
A GoFundMe page to help support her family has been set up online.
