NYPD: Wakefield 16-year-old mother, her 1-year-old daughter missing

Police say a teen mother and her young daughter from the Bronx are missing.
According to the NYPD, police say 16-year-old Reishel Zambrano Chavez and her daughter Zoe Esabella were last seen June 25 leaving their Wakefield home on Bronx Boulevard.
Police say they left unannounced, and never went missing before.
Anyone with information on their whereabouts is asked to call police.
