A new report by the city's Department of Investigation found that New York City Housing Authority officials were warned of problems with a faulty elevator that led to an 84-year-old man's death on Christmas Eve.
Officials say Olegario Pabon died after falling out of an elevator at the Boston Road Plaza Senior Building about 90 minutes after a tenant warned NYCHA of the "very dangerous" situation.
The elevator jerked up while Pabon was riding. His leg and arm were caught in the door, and he fell out and landed on his head, according to the report. Three days later, he died from his injuries.
The report found that the elevator's brake monitor was broken. DOI says NYCHA should have immediately put the elevator out of service, which could have prevented Pabon's death. 
NYCHA say it has taken "unprecedented measures" since Pabon's death to prevent similar incidents.
NYCHA also says it is "aggressively investigating several workers to bring disciplinary action as soon as possible," and that "there must be consequences."
