As temperatures fall and winter approaches, one Hunts Point organization is looking to lift up some of the most vulnerable New Yorkers.
BronxWorks says it's working to aid the "chronically homeless" by tracking down vital documents to help them get income or medical attention.
The end goal is permanent housing, according to Angela Min, a BronxWorks case manager, but there are other ways the organization can help along the way.
Some people living on the streets have no access to their social security cards, birth certificates or other forms of identification they could use to receive benefits or treatment.
