SNAP Benefits Information from Department of Social Services:
For those who don’t want to, or can’t, submit a claim online, they can always go to their local SNAP or Benefits Access Center to submit a physical claim form and/or receive assistance completing the form. They can also access a paper form by either visiting https://www.nyc.gov/site/hra/help/benefitreplacement.page and printing the form; or Calling DSS OneNumber (718-557-1399) and requesting that a paper form be mailed to them. Clients can mail a completed claim form to:

We also conduct in-person benefits replacement events around the five boroughs to help New Yorkers submit claims – we just had one last Thursday at Health Essential Association (2336 86th St Brooklyn, NY 1121) in anticipation of the end of replacement benefits. Will reach out about any upcoming events now that replacement benefits have been extended.
