For those who don’t want to, or can’t, submit a claim online, they can always go to their local SNAP or Benefits Access Center to submit a physical claim form and/or receive assistance completing the form. They can also access a paper form by either visiting https://www.nyc.gov/site/hra/help/benefitreplacement.page and printing the form; or Calling DSS OneNumber (718-557-1399) and requesting that a paper form be mailed to them. Clients can mail a completed claim form to:

We also conduct in-person benefits replacement events around the five boroughs to help New Yorkers submit claims – we just had one last Thursday at Health Essential Association (2336 86th St Brooklyn, NY 1121) in anticipation of the end of replacement benefits.
